For Professionals

With more than 30 years experience working with custom fitted oral appliances as well as additional training in dental sleep medicine, Dr. Thomas Corwin is well suited to provide your patients knowledgeable, experienced care for their obstructive sleep apnea issues.

Who is a candidate for oral appliance therapy?

Patients . . .

  • With mild, moderate, or severe sleep apnea and who are CPAP intolerant
  • With mild or moderate sleep apnea and who prefer oral appliances
  • Looking for an occasional alternative, i.e. for camping, travel, etc.
  • In need of adjunctive therapy, i.e. to use an oral appliance when they remove CPAP in middle of night
  • Looking for a combination, i.e. use an oral appliance to reduce PAP pressure, or to secure nasal pillows without straps

For those patients who struggle with CPAP, or are looking for an alternative, we will make obtaining a properly fitted oral appliance easy for them and for you. Additionally, we are qualified to provide the vital monitoring and management of potential side effects that may be associated with long-term oral appliance use.

Referring to Our Office is Easy

If you have a patient that is in need of oral appliance therapy for OSA or simple snoring, you may call our office to schedule the Sleep Examination appointment. We will need the following documentation from you:

  • A prescription for an oral appliance (E0486). You do not need to specify the type as we will determine the appropriate appliance based on our exam.  Please use our referral form (.pdf) and fax to us Download ›
  • A copy of the diagnostic sleep study for your patient. It may be a PSG or an HST.
  • A copy of the physician notes relevant to the sleep study.
Collaborative Relationship

Dr. Corwin works closely with referring doctors to coordinate our treatment with yours in order to bring to our mutual patients the highest quality care possible. We will provide thorough written communication to keep you informed on patient’s progress with our office. If ever there are any questions please contact us via phone, email or written letter and we will respond as quickly as possible.

Following fabrication and titration of the oral appliance, we will refer your patient back to you for a follow-up calibration sleep study to objectively evaluate the oral appliance’s effectiveness. This is a very important part of successful oral appliance therapy.
